HM Treasury Accepts Procedural Lapses Following Chloe Moffat's Death
HM Treasury has accepted its internal procedures required improvement following the death of former staff member Chloe Moffat. A Surrey coroner, Dr Fiona Borland, issued a Preventing Future Deaths report, detailing shortcomings within the department.
Ms Moffat, 33, tragically died in January 2023. The inquest determined her death resulted from a fall, with the coroner concluding that systemic issues at the Treasury contributed to the circumstances.
Dr Borland's report specifically highlighted failures in recognising and addressing Ms Moffat's welfare concerns, alongside inadequate communication protocols. The Treasury's response indicates a commitment to implementing the coroner's recommendations, including a review of its welfare policies and staff support mechanisms.
The incident underscores broader concerns about mental health support and workplace pressures within government departments, particularly in high-demand environments. Critics argue such situations are often obscured by narratives of efficiency, neglecting the human cost of unaddressed systemic flaws.
