A man discharged from Wrexham Maelor Hospital died two days later from a morphine overdose, after staff mistakenly prescribed the opioid for home use. The Public Services Ombudsman for Wales identified a "series of failures" by medical and pharmacy teams, which significantly increased the risk of a fatal accident. Betsi Cadwaladr University Health Board has since apologised.
Britain's public sector borrowing surged to £24.3 billion in April, the highest for that month since 2020, exceeding expectations.
